Comfort and Care at Home
Hospice services are provided wherever the patient calls home, including assisted living facilities, nursing homes and rehabilitation centers.
Life-limiting illnesses create complex needs – physical, emotional, and spiritual. The Hospice of Moorestown VNA team is experienced in the specialty of hospice care, knowledgeable in the management of symptoms and pain, and skilled in supporting patients and their loved ones throughout the hospice experience.
The caring and professional staff of The Hospice of Moorestown Visiting Nurse Association includes:
- RN’s providing and coordinating patient care
- Social workers providing counseling
- Clergy providing spiritual comfort
- LPNs and Home Health Aides providing physical assistance
- Volunteers providing support and respite
- Telephone assurance and support 24 hours a day, every day
- Bereavement services and support groups to bring comfort to caregivers and those who grieve.
Hospice Care is Available, Affordable, and Accredited…
Hospice care is a covered benefit under Medicare, Medicaid (NJ, PA, DE and most states,) and by most private health insurance plans. Medication, equipment and supplies, related to the admitting diagnosis are also fully covered.
Moorestown Visiting Nurse Association and The Hospice of Moorestown VNA are Medicare/Medicaid certified, licensed by the State of New Jersey, and accredited by the Community Health Accreditation Program.
Referrals for hospice service can be made by the patient, caregiver, family member, or professional. The patient or power of attorney maintains control of all decisions relating to hospice care.
A patient may discontinue hospice care at any time and return to cure-oriented care, a decision that will be respected and supported by The Hospice of Moorestown VNA.
